Outlook Care is one of the largest specialist providers of learning disability services in the Southeast, established since 1990, with an impressive track record in delivering high quality support. We understand that finding the right support or care provider can be a daunting task, and we aim to make this as simple as possible.

By delivering a range of flexible care and support services we will ensure people with learning disabilities are as independent as possible and connected with their local communities.

Outlook Care operate a number of supported living services , A supported living service is a service that helps people with a range of support needs retain their independence whilst still having that extra physical and emotional support needed to lead completely fulfilled lives in their own homes, these services are staffed across a twenty-four-hour period.

A move into supported living can be the first time someone has left their family home or taken a step towards greater independence, and so we're here to provide friendly encouragement and emotional support to make the transition as smooth, positive, and rewarding as possible.

Our customers who choose to live in one of our supported living services are choosing to live independently, this may be in one of our shared supported living services where they will have their own room or in one of our services that provides individual flats .

Staff are on-hand from admission , providing tailored support that helps people to lead more fulfilling lives supported by experienced staff and within an organisation that has operated these types of services for more than thirty years.

Our supported living services are designed to help people live happy lives filled with inclusion and having their wishes and goals understood from the point of assessment , transition, and admission to our services. Our staff will work closely with the customer and their circle of support to ensure that customers are supported to learn new life skills such as cooking or money management, having the confidence to try new things or make new friends. Our staff want to support the customer to work towards life goals this may be volunteering or employment, Our staff are here to support and encourage customers to make positive choices and support them to meet the outcomes that they desire .

Our person centred approach ensures that customers are seen as the individuals they are and each will have a person centred plan that is relevant to their needs , we do include the customers circle of support from assessment through to admission and we ensure that regular reviews take place and the customer outcomes and desires are reviewed and this allows our staff to support the customer consistently.

Our Housing partners are involved from the time that the individual chooses to live at one of our supported living services , we help support them from the beginning to ensure that the process is as smooth as possible. Our housing partners visit the services routinely to ensure that all customers who have a tenancy are fully supported.
