
The support services Outlook Care provides for older people in the community are:
· mental health needs
· learning disabilities
We try to help our service users to do the things for themselves, respect them as individuals and meet their clinical needs.

All people who use our service have a Person Centred Plan.

The plan has information about their:
· social things that they need
· physical needs
· clinical needs
This plan changes as a person's life and needs change. The people who help to write the plan are the:
· person who has care
· persons family
· care team

Our schemes offer a homely feeling and we support and want people to enjoy and take part in the community.

Outlook Care makes sure all its care and support staff are fully trained to know how to support people to enjoy life the way they want.

Our staff out taught about:
· protecting adults who are vulnerable
· how to give people medication
· Person Centred Planning
· Positive Response (physical intervention) accredited with the British Institute of Learning Disabilities
· National Vocational Qualifications in Care
Our nursing staff also attend specialist training so they can form a team of Link Nurses in specialist areas such as:
· Infection control
· Tissue viability
· Management of incontinence
· Manual handling

Our older people's services are registered with the Commission for Social Care Inspection.

The Commission for Social Care Inspection check and report about care services and councils.

Outlook Care offers older people's services such as:
· Registered care
· Nursing care
· Domiciliary care
· Extra care

Supporting People check the way our supported living services are provided.
Supporting People is a programme of help for people who get support and care but are able to do most things for themselves.
